Summary:
A BHT/CC provides direct services to a caseload which may be comprised of various populations under the supervision of Corazon's Clinical Director.
In addition, the Clinician will ensure continuity of care between provider settings and treatment episodes and be responsible for the overall delivery of behavioral health treatment and support services to their assigned caseload.
Primary Duties and
Responsibilities:
Complies with agency, ADHS policy and procedures in the provision of behavioral health covered services and best clinical practices.
When necessary, conducts initial screenings, comprehensive assessments and service plans for individuals referred to Corazon BHS.
Provides communication and coordination of care with a member's family, health care providers, community resources, and other involved supports including educational, social, judicial, community and other state agencies.
Able to assist as needed with completing service plan reviews for all assigned members to ensure that these are done at a minimum of once every 12 months per ADHS guidelines.
Able to assist as needed with completing assessment updates for all assigned members to ensure that these are done at a minimum of once every 12 months per ADHS guidelines.
Maintain up to date case recording (progress notes) for all members on assigned caseload.
Conduct a minimum of one Child and Family team meeting every 6 months on all members under the age of 18 on assigned caseload.
Conduct a minimum of one Adult Recovery Team (ART) meeting every 12 months on all members over the age of 18 on assigned caseload.
Attend a minimumof 4 hours of Clinical Supervision monthly.
Provides outreach activities in order to keep caseload engaged in services and follow-up contacts after any crisis calls or missed appointments and assists with the closure process including engagement and re-engagement activities as needed.
Assures coordination of care and discharge planning activities related to continuity of care between levels of care (i.
e.
inpatient to outpatient care) and across multiple providers if applicable.
